Immigration Medical Exams and Vaccinations for green card applicants.

When you apply for green card or adjustment of status, you are required to undergo an immigration medical examination. The medical examination required by the CDC relations is a means of evaluating the health of aliens applying for entry into the United States. The medical examination must be done by a doctor who is designated as a civil surgeon by the U.S. Citizenship and Immigration Services (USCIS).

Verad Minute Clinic Doctor is designated as a Civil Surgeon, approved by the US Citizenship and Immigration Services (USCIS), and is available to provide services in Tampa Bay.

During the exam, Verad Minute Civil Surgeon will complete a comprehensive medical examination which will include a review of your medical history and physical examination. The Doctor will also test for communicable diseases such as tuberculosis, syphilis and gonorrhea as required by the Center for Disease Control and Prevention (CDC).

Your Paperwork is typically ready for you to pick up within 10 to 14 days after your physical examination, depending on the test that were administered and their results. Verad Minute clinic will complete the I-693 form and seal the form in an envelope for you to submit to USCIS as well as give you a copy for your record.

What to Bring During your Physical Exam

Please fill-out part 1-4 and bring to your examination appointment. Do not sign the form; must be signed in front of your doctor.

Identification

Government -issued photo identification such as valid passport or driver's license, possible form of identification includes your birth certificate.

Vaccination or immunization record

If you are missing some of the vaccinations, Verad Minute Clinic can provide you the immunization at the clinic.
